From Reach to Relevance: The Credibility Crisis Deepens as 82% Execs Claim Effectiveness While Only 28% HCPs Agree

Deloitte's global survey found a staggering 54-point gap: 82% of life sciences executives are satisfied with their current customer engagement strategies, while only 28% of HCPs feel those…

Everyone in pharma commercial is talking about AI investment.

Almost nobody is talking about the gap underneath it.

According to Deloitte's global survey of customer engagement in pharma, "a staggering 82% of life sciences executives are satisfied with their current customer engagement strategies, yet only 28% of HCPs feel that these strategies meet their needs effectively." That's a 54-point canyon between what leadership believes and what the people on the other end of the conversation actually experience.

I don't think that gap is a measurement error. I think it's the most honest number in the industry right now.

The Numbers Behind the Gap

This wasn't a small sample or a one-off poll. Deloitte surveyed "over 190 HCPs and 50 pharmaceutical executives from the top 25 pharmaceutical firms," and the finding was clear: "a significant gap between pharma's outreach efforts and customer needs."

Top 25 firms. Nearly 200 physicians on the receiving end.

Dig one layer deeper and the reason surfaces fast. Deloitte found that "34% of HCPs say that sales reps do not tailor their messages and are ineffective at conveying the scientific narrative." A third of your interactions aren't landing the science. That's not a soft complaint about tone. That's a direct statement that the content isn't doing its job.

It's Not a Reach Problem

Here's where most teams get it wrong.

The instinct is to add more channels, more touchpoints, more frequency.

But according to Diaceutics, "97% of digital outreach goes unanswered." Ninety-seven percent of digital outreach goes unanswered. If the problem were reach, more volume would break through eventually. It isn't breaking through. The problem sits somewhere else.

Pharma already tried the volume play. SciencePOD reported that "pharma produced 29% more content in 2025 than the year before." Nearly a third more output, year over year. If more content solved this, the 28% would be climbing. It isn't.

At industry gatherings in 2026, the question stopped being whether to invest and started being whether any of it was working. As SciencePOD put it, "pharma has never produced more content. It has never invested more in AI. And it has never been further from solving the fundamental problem: making its messages land with the people who need to hear them."

More content. More AI. Less connection.

That should stop every commercial leader mid-sentence.

The Spend Doesn't Match the Outcome

Let's be specific about the scale of that investment.

SciencePOD cited an "estimated global investment of $250 billion in 2024 alone" in AI across the industry. That figure represents significant capital deployment, but the HCP satisfaction number still sits at 28%.

I'm not arguing against AI investment. I'm arguing against the assumption that investment alone closes a trust gap. Capability isn't what wins here. Relevance is.

If a rep walks in with a beautifully generated leave-behind that still doesn't answer what that physician actually needs to know, the technology hasn't failed. The judgment behind it has.

Naming the Real Problem: Relevance Debt

Made by XDS gave this condition a name. Their analysis states plainly that "that 54-point spread is not a channel gap or a targeting gap. It is a content-quality gap, or what we call relevance debt, that has compounded from years of prioritizing volume over usefulness."

Relevance debt.

It accumulates quietly. Every generic message, every untailored call, every piece of content built for a persona instead of a person adds to the balance. You don't see the bill until the HCP stops opening emails, stops taking meetings, stops believing the rep has anything new to say.

Deloitte's finding that "34% of HCPs say that sales reps do not tailor their messages and are ineffective at conveying the scientific narrative" and Diaceutics' 97% non-response rate aren't separate problems. They're the same debt, compounding across two stages of the same conversation.

Where Signal Intelligence Fits

This is exactly the diagnosis that matters to us at ReflectivAI.

Closing a 54-point perception gap doesn't start with another content push. It starts with seeing what actually happens inside the conversation. What a rep notices, how they interpret it, how they respond in the moment. That's the layer most strategies never measure.

Territory design, call planning, message sequencing. All of it optimized. None of it tells you whether the judgment in the room was any good.

We're building toward something we call Signal Intelligence. Human-led. Governed. No claims about clinical outcomes or regulatory impact. It's about making the quality of a conversation, not just the volume of outreach, something you can actually see and improve.

The Open Question

That 82%/28% gap isn't going to close with a better dashboard or a new AI model bolted onto the same pipeline.

It closes when the industry stops measuring reach and starts measuring relevance, conversation by conversation.

The $250 billion is already spent. The 29% more content is already out there.

So here's the real question for commercial leaders: Is anyone actually watching what happens the moment a rep and an HCP are in the room together? And is that moment getting better, or just getting louder?
