The HCP Content-Quality Crisis Deepens: 54-Point Credibility Gap Locked by Relevance Debt, Not Channels
A Deloitte global survey of over 190 HCPs and 50 pharmaceutical executives from the top 25 pharma firms found 82% of life sciences executives satisfied with their current customer engagement…
Everyone is talking about channel mix. Very few people are talking about the thing HCPs are actually rejecting: the substance of what shows up in their inbox, their portal, their rep's tablet.
Pharma keeps investing in delivery. HCPs keep saying the content itself doesn't fit them.
That gap has a number now. It's 54 points.
The Gap, By The Numbers
A Deloitte global survey of more than 190 HCPs and 50 pharmaceutical executives from the top 25 pharma firms found something uncomfortable. According to Deloitte Switzerland, "a staggering 82% of life sciences executives are satisfied with their current customer engagement strategies, yet only 28% of HCPs feel that these strategies meet their needs effectively." Eighty-two versus twenty-eight. That's the 54-point gap in the headline, and it isn't a rounding error.
Deloitte's 2025 Life Sciences Outlook confirms the same split from a different angle. "Over 80% of pharma executives said they are satisfied with their current customer engagement strategy," while only about a third of HCPs agree the content actually serves them.
Two separate Deloitte reports. Same disconnect. That's not noise. That's a pattern locking into place.
It's Not The Channel. It's The Relevance.
Here's where most teams get the diagnosis wrong. They assume the fix is more channels, faster cadence, better targeting software.
But Deloitte's own data points somewhere else. In the same survey, 34% of HCPs say that sales reps do not tailor their messages and are ineffective at conveying the scientific narrative. That's not a delivery problem. That's a judgment problem. Reps are showing up. The content isn't showing up for the physician in front of them.
And HCPs have told pharma what they actually want. Deloitte found 67% of HCPs believe pharma engagement should drive high patient awareness about health issues. Not volume. Not frequency. Usefulness.
Executives are optimizing for reach. HCPs are asking for relevance. Those are not the same project, and treating them as the same project is how you get an 82-versus-28 split that nobody planned for.
Fatigue Has A Number Too
If the tailoring problem sounds soft, the fatigue data makes it concrete.
Indegene's Digitally Savvy HCP survey, covering nearly 1,000 HCPs across 20+ specialties and eight countries, found that "62% are experiencing product-related promotional content fatigue and 70% feel that pharma representatives do not understand their requirements completely." Seven in ten HCPs believe the person or system talking to them hasn't actually understood them.
That's not a content-volume complaint. It's a comprehension complaint.
Then there's the trust layer sitting on top of fatigue. A Graphite Digital survey of consultant-level oncology and urology HCPs in the U.S., U.K., and France found "80% of consultants are skeptical of pharma content because of doubts about the cherry-picking of data and other concerns," as reported by Fierce Pharma.
Stack these together. HCPs are overwhelmed by content, doubt the reps delivering it understand them, and distrust the data behind it. That's three compounding failures, not one.
HCPs Aren't Waiting Around
Here's the part that should worry commercial leaders more than the gap itself.
HCPs aren't sitting passively inside broken engagement models. They're routing around them. Reporting from MM+M notes that HCPs are using tools like ChatGPT at rates comparable to search, but still validate against journals, guidelines and clinical databases before making decisions.
Read that carefully. AI isn't replacing verification. It's becoming a filtering layer HCPs use to triage what pharma sends them before they trust it enough to check further. If the initial signal doesn't clear that filter, it never reaches the verification stage at all.
Meanwhile, the channel investment keeps flowing into digital. More digital volume continues to flow without consistently closing the experience gap that HCPs report. The evidence suggests that digital-first strategies alone haven't closed the relevance problem, even as pharma has shifted significantly toward digital channels.
What This Actually Means For Commercial Teams
Put the pieces next to each other and the diagnosis gets simple, even if the fix isn't.
Executives are satisfied because they're measuring delivery. HCPs are dissatisfied because they're judging relevance. Those are different scoreboards, and pharma has been optimizing the wrong one.
The judgment that matters happens inside the conversation itself. Did the rep notice what this physician actually needed right now? Did they interpret the signal correctly? Did the response match it?
That judgment has been invisible. Measured by call counts and content downloads, not by whether the interaction actually landed.
At ReflectivAI, we think that judgment can be made visible, without turning it into another surveillance layer or a black-box AI score nobody trusts. That's the premise behind Signal Intelligence: noticing, interpreting, and responding as behavior you can observe and govern, human-led, not a proxy metric standing in for it. We're early. This is pilot and roadmap work, not a finished system, and I'd rather say that plainly than oversell it.
The 54-point gap didn't open because pharma stopped showing up. It opened because showing up stopped being the same thing as being relevant.
So here's the question worth sitting with: if your team is confident in its engagement strategy, whose scoreboard is that confidence based on—yours or the HCP's?
